I wish I had better optics for pictures and this is the best I can offer right now. I do not believe this to be MD, die fatigue etc. etc. etc. Its some of the better doubling I've seen in a while. Maybe wrong(most likely) but I think its a hubbing variety. You be the judge. Again I'm sorry I don't have better pics. Its most prominent in PEN

Since that lettering is incused, its definitely machine doubling just like the doubling in the attachment is. Serif splits and all.
James Wiles explained it this way:
"The lettering around the border field is incuse. MDD on incuse design elements will show splits in the serifs. While true die doubling will show as extensions."
When asked "Extensions?", I was referred to the web page below for an example of what a true "incused lettering" doubled die looks like.
