1924 Peace Dollar with large neck "bump" and reverse die pitting???

This coin looks circulated, so it is difficult to say what happened to it. Only if these coins could tell us the tale of their travels would we truly know what happened to them.
On the obverse - is any part of that bump sink into the coin ? Could someone have used a circular punch to make an "O" on her neck?

I have seen an article about this condition before. It was about Lincoln cents and I THINK it was about hardness or strike testing. Mike Diamond hangs out in the Mint Errors side of the forum.
