  • 1996 Quarter PMD or mint error?

A friend of mine at work brought me this to see if it was an error or just damage. I have never seen this before so I would like some opinions. It is only about 75% as thick as it should be. The obverse is normal but look at the pics of the reverse and tell me what you think. Thanks!

Post-strike damage. The reverse face was machined away.
