DDO and DDR (MD, bad die, bad alloy?) on 1976 (P or D?) Bicentennial quarter

Hi - So I have this Bicentennial quarter that might be doubled on the obverse and reverse. There is a 1976-D DDO FS-101. However, I don't see a mint mark on this one. If this is DDO, maybe the mintmark has eroded or been mis struck? Then again, I'm a newbie whose been a casual researcher for years until recently.

The most glaring example of potential doubling is in the "T" on Liberty. The "U" in United looks a little like another U was struck lower against it. In fact, from some angles, it seems like the "ghost" of another word travels across and halfway up United. The drummer boy's hand seems oddly doubled (and there's a large divot - could be damage, of course.) Also, at about 4:00 on the obverse, it looks like something might have been struck there.

Attached are a number of pics of the quarter in question, along with some shots that point out a couple of the anomalies I mentioned above. All input welcome. Thanks!
