Here are several quarters with dark surfaces. Maybe lamination errors or annealing errors?

They simply look discolored. Whether this occurred inside or outside the Mint is impossible to say. The streaks on the reverse of the North Carolina quarter are almost certainly something that developed during the strip/planchet preparation process, although there is disagreement about where, when, and how they are generated.
