It looks like it was exposed to some sort of skidding. The obverse was face down more than likely.
It may have been on a cement or tile floor, got wedged between the floor and a wheel of a chair or something like that and skidded a short distance.
Possibilities are endless. I don't see this as an error, to me it looks more like damage.Three helpful posts:
