Mostly machine doubling, maybe a little die wear. Not a RPM - sorry.

It has NOTHING to do with hub doubling. There is die wear, machine doubling - basically a normal coin.

This looks like hubing to me . see how the letters are pulled. plus double punched you can see that.

It's all the same, not a doubled die.

Agree completely with Jason here. I think you are also confusing hubbing with the striking of the coin Rocky. You are calling it punching, but it is striking of the coin. If you are not completely up on the hubbing process of US coins, let me know and I will get you a few links to look at or you can search for it yourself. Technically, there is no way hubbing of a die will result in anomalies like this.
