1996 P Washington Quarter
This is one of my favorite error quarters that I've come across coin roll hunting. It's a MAD, and on the reverse there's the normal five arrow tails in the left Eagle claw but, only four arrows in the right claw.

It is definitely a misaligned die (MAD) but not an obverse off center strike. (it can be one or the other...not both). As far as the reverse anomaly you are pointing out, the 5th arrow tip is there on top of the bundle. It just seems that it didn't strike up completely.Bob Piazza
