There are numerous reasons. A couple are that continues striking has caused some die deterioration, and a chip forms inside the mint mark. Circulation damage may cause this, as well as purposeful die damage. Since mintmarks were hand punched up until 1990, incorrect punching by the mint technician may also be a reason. Some mint marks were punched so strongly that it lifted up some metal inside the mint marks. This is more prevalent with the S mint marks.Bob Piazza
