1996 quarter error?

I believe it is a mis-aligned die error. I was hesitant to acknowledge since the reverse, the rim to the south is a bit wider, but that obverse, undeniably has issues.

I normally don't run into these with the MAD being that far off. It is pretty good for a MAD.
